Copy of Supporting Document Checklist

The Supporting Document Checklist indicates all the required supporting documentation that the San Francisco Redevelopment Agency (SFRA) will need in order to consider any application complete. 

Income Asset Disclosure Form

All applicants are required to fill out this form and submit it with their supporting documentation in order to have a complete application.

Verification of Employment

If an applicant is not able to provide pay stubs they will have to have their employer fill out this form. Other applicants that may receive a request by a HomeBricks Certification Specialist for the Verification of Employment are applicants who have pay stubs that do not have a time period printed on them, or for anyone who has pay periods other then 12, 24 or 26.

Verification of Non-Filing

If an applicant is not able to provide a tax return due to non-filing for any reason over the past three years it is required to get a letter verifying this from the IRS the above form must be filled out and submitted to the IRS to obtain this verification of non-filing.

Lottery Number Suspension Request Form

This form is required for any applicant wishing to seek a higher purchase amount.

Tips:

Applicants should not submit any documents to HomeBricks until they are requested. 

If an applicant is planning to be out of town they should let there HomeBricks Certification Specialist know ahead of time so that no deadlines will be missed and the applicant will not lose their place in line.
